Actors Release Form

TorahFlix Actor Release Requirement

To appear in any TorahFlix production, all participants must submit a signed actor release form:

• Adults (18 and over): Must sign their own release form.
   
• Minors (under 18): A parent or legal guardian must sign on their behalf. Minors cannot sign for themselves.
   

This form grants permission to use your image, voice, and likeness in TorahFlix media projects.

Please submit the signed form before filming.
